If you realised that that is an error in your marriage registration, the Jabatan Pendaftaran Negara (JPN) requires you to fill and submit the KC 08 form in order to rectify the error.

The form is known as Borang Pembetulan Kesilapan.

Inside the form, you are required to fill in the following information:

1) Marriage registration details (Maklumat daftar perkahwinan)

2) Correction details (Pembetulan maklumat-maklumat yang di kehendaki)

3) Supporting documents (Dokumen sokongan)

4) Declaration (Akuan pemohon)
