What is the purpose of the Jabatan Pendaftaran Negara (JPN) KC09 form?

The KC09 is needed if you want to search or extract the marriage registration information of the people who have registered with JPN.

Remember, not everyone can do a search for marriage registration information on JPN.

Only a spouse or their children or parents or siblings (brother or sister) or an appointed lawyer with an authorised letter may do the search.

The JPN KC09 form is known as Borang Permohonan Carian / Cabutan Daftar dan Dokumen Perkahwinan.

Inside the form, you are required to fill in the following information:

1) Husband / wife information

2) Focus of search

3) Applicant’s information and declaration
