If you’ve bought vehicle insurance from Allianz Malaysia Berhad and you had a car accident, you can claim from your insurance company by submitting a claim form.

In order to claim your vehicle insurance, first, download the Motor Accident Report form.

You would have to fill the following information into the form:

• Types of claim – own damage, theft of vehicle, windscreen damage, claim against third party, injury to thrid party or damage to third party vehicle
• Particulars of injured
• Insured GST details
• Particulars of the drivers
• Particular of accident / theft

You are also required to sign the Data Privacy Notice & Consent Form (Death Claims).
