Before you are allow to drive a vehicle in the state of Arizona, you are required to fill and submit the Arizona Driver License / Identification Card application form.

The form is issued by the Arizona Motor Vehicle Division.

You are required to fill the following information into the form:

• The type of license applied – driver license, motorcycle license, instruction permit, identification card, travel driver license, travel identification card
• Social security number
• Applicant’s personal information
• Mailing address
• Driving history
• Vehicle registration information
• Medical information
• Voter registration
• Organ / tissue donor declaration
• Military veteran
