In Bahasa Malaysia, it is known as Borang Permohonan Sijil Taraf Mastautin.

The information required to be filled in the form are:

• Name of taxpayer
• Income tax reference number
• Principal business activity
• Treaty country
• Year of assessment required
• Type of income received
• Period of payment
• Amount of payment received
• Name and address of foreign partner
