The Jabatan Pengangkuatan Jalan (JPJ) TM-1 form is used for the application of Transfer of Vehicle Ownership voluntarily.

In Bahasa Malaysia, the document is known as Borang Permohonan Pertukaran Milikan Kenderaan Motor Secara Sukarela.

The form will require the following:

• The applicant’s personal details
• JPJ K3 form
• JPJ K2 form
• JPJ TM-AB form if applicant not present during document submission
